               Registration Information for
         Long Valley Junior Women’s Club 
             2010-2011 C.A.R.E. Program

Registration will take place at Old Farmer’s Road School from 7:00 to 9:00 pm in the gym.  Registration will be on a first come, first serve basis regardless of school.  Each individual must register only one family.  CARE does not allow one individual to register multiple families. 

REGISTRATION DATES are:

Tuesday, June 8, 2010 -- for families Currently enrolled in the 2009-2010 C.A.R.E. Program

Thursday, June 10, 2010 -- for New Students to the C.A.R.E. Program

What to bring to Registration:

2 Completed Registration Forms for each class, per student

Must have all Emergency Contact Information completed on form:

Doctor’s name/phone #

Dentist’s name/phone #

Emergency Contact name/phone #

Payment (check is recommended) – Payment in full is recommended but we do offer the option of paying by installment plan.  See above for Payment Options. 

                   NO CREDIT CARDS ACCEPTED!!
